Sport Obligations
To participate in an athletic activity the following forms need to be completed and returned to the main office. There are also fees associated with each sport.
A physical form is filled out by a physician and parent and needs to be updated every other year. The year that a physical form in not needed an alternate year form needs to be filled out by the parent.

Code of Conduct Meeting
Incoming Freshmen and a parent or guardian are required to attend a Code of Conduct meeting before they will be able to participate. There will be a QR code at the meeting that they will scan to prove they attended one of the meetings. Dates and locations will be sent out at the end of each school year.
