Sport Obligations
To participate in an athletic activity the following forms need to be completed and returned to the main office. There are also fees associated with each sport.
A physical form is filled out by a physician and parent and needs to be updated every other year. The year that a physical form in not needed an alternate year form needs to be filled out by the parent.
ImPACT Consent
The purpose of this form is to confirm receipt of the Reedsburg Area High School “Concussions and Head Injuries” policy and other information relating to concussions and head injuries that may occur during participation in athletic programs and to provide the Reedsburg Area High School permission to administer the ImPACT Assessment on my student.
1) Good for two years, typically needed for Freshman, Juniors and new athletes
2) Needed for the following sports: Baseball, Basketball, Football, Gymnastics, Hockey, Soccer, Softball, Volleyball, & Wrestling.
Pledge Summary
The pledge form is signed by both the student and parent acknowledging to adhere to the code of conduct policy. Pledge forms only need to be filled out once a school year.
